To register for the 2008/2009 school year, please download and fill out our registration form and enrollment documents and return them to the Arlington Enrichment Collaborative with payment. We are required by law to have completed enrollment forms for every student that attends our program. The following list explains each form and its purpose.

- Enrollment Form – this is the standard enrollment form
- Registration Form – this tells us what days your student will be attending AEC
- Emergency Information – this gives us all of the contact information we need should we need to contact you for some reason. It also gives us necessary information on your student.
- Permission and Releases – there are 4 signatures required on this form for allowing us to take your student off premises, to allow us to communicate with school about things that may effect your student’s schedule or school work, a media release that allows us to use pictures of your student for our posters, website, etc. and permission for your student to leave the premises unsupervised.
- Parent and Student Expectations – here you can tell us about your student and any special concerns you have as well as your expectations for the time that they spend with us at AEC.
- Transportation Plan – this tells us how you plan to have your child arrive at and depart from AEC.
- Medication Form – this gives us permission to dispense prescription and over the counter medication to your student. You will need to fill in one of these per medication.
In addition, we now require these two forms for students to leave the premises unaccompanied by staff:

- Parent/Guardian Authorization
All 9 of the above documents must be returned with a first payment before your child's first day at AEC.
